After 25 years of of proudly serving our community, we are transitioning our Provider Services to other organizations.

Michele Lew, CEO, shared the news in THIS recent message. We are presently coordinating with partnering organizations and agencies that can continue to provide resources for our clients. We will be sharing additional information as it becomes available.

Food & Nutrition

We are pleased to be partnering with the following organizations for your ongoing nutrition support. Clients that returned their ROI, received updated information about their new provider. 尤物社 nurtrtion services end in June but if you need support, the agencies listed below may be able to serve you. Additionally, we have shared a flyer with many similar options for food resources.

Loaves and Fishes 

MISSION: L & F aims to provide hot and nutritious prepared meals, delivered and served to feed hungry families, children, seniors, veterans, students, and disabled individuals in our Bay Area community. All are welcomed, no questions asked.

Sourcewise

MISSION: Sourcewise provides adults and their caregivers the tools and services they need to effectively navigate their health and life options. Through a comprehensive network of resources, Sourcewise strives to educate, prepare, support, and advocate for all adults, their families, and their caregivers within Santa Clara County.

POSSO

MISSION: The Portuguese Organization for Social Services and Opportunities is committed to the enrichment of the quality of life of all members of the community. Opportunities for friendship, service, and lifelong learning are essential to live a full, productive life in the mainstream of society.


Free Grocery Day@ Tropicana Plaza in East San Jose will be closed for Summer and reopen in the Fall of 2024

Chronic Disease Prevention & Management

Our chronic disease prevention and management services are transitioning to the following agencies listed below. Clients of 尤物社 were invited to a Town Hall to ask questions and the Q&A from that meeting is available to view HERE.

Clients receiving housing support can contact the following agencies for continued assistance. 

Housing Plus Project (HPP)

Bay Area Community Health (BACH) will take over this service. Appointments will be required. Learn more at the link.

Housing for Health

This service will transition to the City of San Jose. Please reach out to Christine Caldwell 408-278-6483 OR

H.O.P.W.A.

This service will transition to the City of San Jose. 尤物社 issued rent coverage for July and August to landlords in June 2024. If you need assistance please contact Nolan Tran nolan.tran@sanjoseca.gov

Ryan White Program Services

Case Management, Medical Transportation, Emergency Financial Assistance, and Safeway Gift Cards: These services will transition to Public Health, located at . Clients can expect to hear from them before their next re-enrollment date, if not sooner. Once Public Health has contacted you and you confirm you would like to receive case management services from them, your client information will automatically transfer to your new case manager. Appointments will be required for most services. Learn more at the link.

The Jerry Larson Food Basket

Loaves & Fishes Family Kitchen will take over this service, continuing at 1043 Garland Ave, San Jose. Loaves & Fishes is a non-denominational organization with no religious ties, welcoming all individuals without any questions. They are committed to ensuring that those in need of a meal have access to nutritious food. Your client information can be transferred to Loaves & Fishes if you choose to provide them with your consent form. You should receive a calendar with July Food Basket dates and times at the last Jerry Larson Food Basket on June 24-25, 2024. Learn more at.

尤物社’s Housing Services have transitioned to county-based agencies.
