After 25 years of of proudly serving our community, we are transitioning our Provider Services to other organizations.

Our dedicated team has coordinated with partnering organizations and agencies to takeover direct service programs for our clients; we are committed to a smooth and seamless transition, with little to no disruption to community members. Information on each program’s new home is available below.

Food & Nutrition

If you are a former client of ÓÈÎïÉç receiving services through our Meals on Wheels, Medically Tailored Meals, or Jerry Larson Food Basket programs, please check your mailbox for communication from your new service provider. If you have not received notification and need assistance connecting to your new provider, please contact info@healthtrust.org.

We are pleased to partner with the below organizations for your ongoing nutrition support:

Meals on Wheels & Medically Tailored Meals – Loaves & Fishes 

Loaves & Fishes Family Kitchen will serve clients through Meals on Wheels (MOW). Medically Tailored Meals (MTM) and Grocery services for Medi-Cal and Dual-Connect clients with Santa Clara Family Health Plan insurance will also be provided by Loaves & Fishes. MTM clients can call Loaves & Fishes at 408-961-9880 x140 or email mtm@loavesfishes.org. For clients with Kaiser or Anthem insurance, please contact your insurance directly:

Meals on Wheels – Sourcewise

Meals on Wheels will also be delivered by Sourcewise. Sourcewise provides adults and their caregivers the tools and services they need to effectively navigate their health and life options. Through a comprehensive network of resources, Sourcewise strives to educate, prepare, support, and advocate for all adults, their families, and their caregivers within Santa Clara County.

Meals on Wheels – POSSO

The Portuguese Organization for Social Services and Opportunities (POSSO) will also provide community with Meals on Wheels. POSSO is committed to the enrichment of the quality of life of all members of the community. Opportunities for friendship, service, and lifelong learning are essential to live a full, productive life in the mainstream of society.

Chronic Disease Prevention & Management

Our chronic disease prevention and management services are transitioning to multiple agencies. If you are a former Health Trust client receiving Chronic Disease support, please check your mailbox for communication from your new service provider.

We are proud to partner with the following organizations to offer you continued support:

Ryan White Program Services

For Case Management, Medical Transportation, Emergency Financial Assistance, and Safeway Gift Cards

These services will transition to Santa Clara County’s Public Health Department. Clients can contact RyanWhite@phd.sccgov.org or 408-792-5080 for more information.

Housing Opportunities for Persons with AIDS (HOPWA) & Permanent Supportive Housing (PSH)

Clients assisted with subsidies through HOPWA and PSH will be served by the City of San Jose. Clients with questions can reach out to Nolan Tran at nolan.tran@sanjoseca.gov.


Housing Plus Project (HPP)

For HPP clients, Bay Area Community Health (BACH) will be your new provider agency. Appointments will be required. Please contact Gloria Preciado at 510-410-7397 and gpreciado-santana@bach.health or Aliaa Bilal at 510-294‑9258 and abilal@bach.health.

The Jerry Larson Food Basket

Loaves & Fishes Family Kitchen will take over operations of The Food Basket starting July 1, 2024. The Food Basket will remain at 1043 Garland Ave, San Jose and can be reached directly at 408-380-8620.


Clients of ÓÈÎïÉç were invited to a Town Hall to ask questions, and the Q&A from that meeting is available to view HERE.

Housing Services

ÓÈÎïÉç’s Housing Services programs have transitioned to multiple agencies. If you are a former Housing Services client of ÓÈÎïÉç but do not know which program you were enrolled in, please call 408.961.9805email HousingServices@healthtrust.org for assistance connecting to your new provider.
